Community Connector (CC)

Program Overview

The Community Connector (CC) program at Hope Family Care Community LLC supports individuals with intellectual and developmental disabilities in navigating and engaging with their communities. Services focus on building relationships, strengthening natural supports, and encouraging safe, independent participation in everyday activities. CC is designed to support personal goals while promoting confidence and community inclusion.

How Community Connector Services Work

Community Connector services are delivered in typical community settings, providing real-life learning experiences. Support is offered on a one-to-one basis to maximize individualized guidance and skill development. Services may be provided as a one-time experience or on a recurring schedule, depending on individual needs and preferences.

Individuals may gain the following:

• Community Access and Skill Building
  Through CC services, individuals gain hands-on experience accessing community resources competently and safely. Support includes exposure to cultural, social, recreational, volunteer, and future employment-related opportunities available within the community. Individuals also receive guidance on how to navigate community environments in an informed and confident manner.

• Personal Choice and Development
  The Community Connector program encourages individuals to actively plan activities, make choices, and develop skills while engaging in their communities. Services emphasize self-direction, allowing individuals to explore interests and build meaningful connections at their own pace. This approach supports long-term independence and ongoing community involvement.
