What types of services do you provide?
We offer a range of developmental disability support services, including Individual Residential Services and Supports (IRSS), Supported Community Connections (SCC), Community Connector (CC), Mentorship, and Transportation Services. Each service supports independence, safety, and meaningful community participation. Services may be delivered individually or in small group settings, depending on the program.
How are services planned and customized for each individual?
Each individual leads the development of their own person-centered plan with guidance from our team. Services are tailored to reflect personal goals, preferences, routines, and support needs. Plans are reviewed regularly to ensure services remain effective and responsive.
Where are services provided?
Services are delivered in community-based settings, including individual homes, shared residential environments, and typical community locations. Depending on the service, support may take place in residential settings, public spaces, workplaces, or community programs. Individuals choose environments that best support their comfort and goals.
What happens if a service or living arrangement is no longer a good fit?
Individuals are encouraged to reassess their services as needs or preferences change. If a service model or setting no longer meets expectations, we support a smooth transition to ensure continuity and informed choice.
