Individual Residential Services and Supports (IRSS)

Program Overview

Individual Residential Services and Supports (IRSS) at Hope Family Care Community LLC provides around-the-clock assistance for individuals with intellectual and developmental disabilities (IDD) who wish to live as independently as possible. Services are designed to support individuals in community-based settings of their choice while maintaining health, safety, and personal autonomy. Each living arrangement is tailored to align with the individual’s goals, preferences, and level of support needed.

Living Arrangements

IRSS is delivered in small, supportive residential settings serving three or fewer individuals who live together. Individuals may reside in a home or apartment they own or lease, a residence owned or leased by the agency, or a Host Home environment. These flexible options allow individuals to live in settings that feel comfortable, familiar, and supportive of independence.

Our living arrangement models include:

• Personal Care Alternative (PCA) Model
  Hope Family Care Community LLC offers Personal Care Alternative (PCA) options designed for one to three individuals living together. PCA settings provide structured daily support within environments that can be customized to meet each person’s unique needs. These settings may include an individual’s own home or an agency-managed residence, ensuring flexibility and continuity of care.

• Host Home Model
  The Host Home model provides individuals with the opportunity to live in a private family residence with a dedicated caregiver, couple, or family. Host Homes typically support one to two individuals, with three-person arrangements considered when appropriate and agreed upon by all individuals involved. This model promotes stability, community inclusion, and independent living within a family-oriented environment.

Individuals may gain the following:

• Service Planning and Ongoing Support
  Hope Family Care Community LLC leads the development of each individual’s Individual Services and Supports Plan (ISSP) and actively participates as a core member of the Interdisciplinary Team (IDT). Service quality is reviewed monthly to ensure consistency and effectiveness. Supports include 24-hour on-call assistance, nursing oversight, medication administration, dietary accommodations, accessible housing, transportation to medical appointments, and access to community-based services.

• Personal Choice and Transitions
  Individuals are encouraged to select the residential model and living environment that best meets their needs and personal preferences. If available PCA or Host Home options are not the right fit, Hope Family Care Community LLC supports individuals through a smooth transition to another provider, ensuring continuity and informed choice at every step.
