Program Overview

Supported Community Connections (SCC) at Hope Family Care Community LLC helps individuals with intellectual and developmental disabilities actively engage in their communities. Services are designed to support relationship-building, natural supports, and meaningful participation in everyday community life. SCC promotes independence and safety while supporting individual goals.

How SCC Services Are Delivered

SCC services take place in typical community settings, creating real-world learning experiences that support confidence and independence. Services may be provided as a one-time experience or on a recurring basis, depending on individual needs and goals. Support is offered through one-to-one assistance or small group settings with two to five participants.

Individuals may gain the following:

  • Community Participation and Skill Development
    Through SCC, individuals gain hands-on experience navigating their community in a safe and informed manner. Services expose individuals to a variety of cultural, social, recreational, volunteer, and employment-related opportunities available in their area. This approach supports skill development, social engagement, and informed decision-making.

  • Individual Choice and Personal Growth
    Individuals are encouraged to plan activities, make choices, and build skills while accessing community resources. SCC emphasizes self-direction by allowing individuals to explore interests and develop connections at their own pace. These experiences help strengthen confidence, independence, and long-term community involvement.
